- Among 163 surgical patients, mixed organizing and organized thrombus was associated with lower adjusted odds of residual pulmonary hypertension and a larger postoperative pressure reduction than organized thrombus alone.
Limitations
- Histopathology is available after surgery, so the result is not yet a preoperative selection rule.
- Retrospective association cannot show that timing based on thrombus stage improves outcomes.

- Across 81 VV-ECMO consultations, 44 people were judged noncandidates, 29 candidates, and eight decisions were deferred.
- Fifteen contraindications were used inconsistently: 26 of 29 candidates had at least one feature that had been prohibitive in a noncandidate, and contraindications documented in some noncandidates were present but unmentioned in 21 others.
Limitations
- This was one tertiary referral center.
- Comparing recorded rationales can expose inconsistency but cannot determine that every different decision was wrong, establish causal bias, or show which standardization strategy improves patient outcomes; only nine candidates were cannulated.
